To apply for Medicaid benefits in Alabama,
you must fill out the correct form(s) and return
them to Medicaid in person, by mail, or online. A list of most frequently used applications
and forms is below. If you want to apply for
SSI/Medicaid due to disability you may apply at
your local Social Security Administration
office. If you cannot find the information you
need, contact 1-800-362-1504.
Please note that the Alabama Medicaid Agency
does not accept faxed applications.
